100-year-old undergoes a successful ERCP with CBD stenting

 A 100-year old lady got a second lease of life after she underwent a successful ERCP with CBD stenting at Holy Family Hospital.

THE CASE:

The patient was brought to Holy Family Hospital with choledocholithiasis (presence of stones in bile ducts) causing cholangitis (bile duct infection and inflammation). Common bile duct is a 7-8 mm small luminal structure that transports bile (digestive juice) from the liver to the duodenum. In the case of this patient, it was blocked by multiple large stones, leading to jaundice and infection which was spreading in the body, further deteriorating her condition. The patient was in septic shock on presentation and was admitted to ICCU and was on medication to stabilize her vital parameters,

THE PROCEDURE:

The only lifesaving solution to this situation was to do a therapeutic ERCP (endoscopic retrograde cholangiopancreatography) which is an advanced, intricate endoscopic procedure where the duodenal end of the bile duct is precisely cut endoscopically from inside and a stent is inserted in the duct to relieve the block. The procedure was undertaken by Dr. Darshan Parekh, a consultant Interventional Gastroenterologist at Holy Family Hospital. The challenge was to do it very meticulously and fast with minimal anesthesia attributing to her old age and medical condition. The procedure was completed in approximately 15 minutes.

THE RESULT:

Because of the strong will power of the lady and positive attitude of her relatives, the patient was shifted to the ward from ICU in 2 days. She had an uneventful recovery and was discharged on 11" May 2022.
